Castle Peak,a senior living community in Eagle, CO, is hiring aFull-Time Director of Social Servicesto join our team at our beautiful building.
The Director of Social Services will advocate for our residents, families, and staff and provide overall leadership and management.
Our ideal candidate thrives in a fast-paced environment, excels in multitasking, and has excellent customer service skills.
In addition to generous benefits, we are offering a $5,000 Sign-On Bonus!This is aFull-Time, benefit eligible position working normal business hours from Monday to Friday.
Pay:
$62,400 to$100,000 / year depending on experienceDirector of Social Services
Responsibilities:
Provideguidance and direction to the social services department through policy and procedure creation and monitoring to ensure the Cassia mission is being realized.
Coordinate caseloads, complete social history, care plans, MDS, and discharge plans for new residents.
Engage with community partnerships.
Participatein resident care conferences involving the planning and implementation for medically related social services, mental healthservices, and emotional support services.
Oversee and complete referrals for resident mental health services and coordinate consultative services with providers.
Complete social servicesadmission evaluation, as well as discharge, care plans for residents.
Maintain necessary documentation in resident's medical records.
Perform other duties as assigned.
Director of Social Services skills and requirements:
A qualified Social Services Director is a person who is either:
A social worker with a Bachelor's degree in social work registered or authorized expressly by Colorado law to practice as a social worker who has one year of social work experience under the supervision of a licensed social worker in a health care setting working directly with residents, or A social worker with a Master's degree in social work who is licensed as a licensed social worker or licensed clinical social worker under Colorado law, or A person with a Master's or Bachelor's Degree in a related human services field who has monthly consultation with a person meeting the qualifications in subsection 2 above.
The consultation shall be sufficient in amount to assist the social work staff to meet resident needs.
LTC experience preferred.
Prior experience in the admission process, discharge planning, or management preferred.
Strong knowledge of local, state, and federal guidelines related to long-term care and resident rights.
Strong leadership, written and verbal communication skills.
Good communication and the ability to multi-task.
Bi-lingual English/Spanish preferred

The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
